The first step to restoring your driver’s license is completing an assessment. If you need an assessment done immediately, we can also often accommodate walk-in appointments.

There is a state-regulated assessment fee of $110 (this amount should be the same at all DWI Assessment Providers).

Charged Out of State

At Smart Choice, we’ve worked with many different state government agencies to complete out-of-state assessments and take pride in going the extra mile to see that all of your required paperwork is completed accurately and on time.

There is a $150 assessment fee (same fee as if you were charged in North Carolina). Once your assessment is completed, we’ll also complete any additional paperwork that’s needed if your care needs to be coordinated with an out-of-state agency.

Out-of-State Reviews (charged in NC but live out of state)

An Out-of-State Review is needed if you live a considerable distance from North Carolina but were charged or convicted of a DWI in North Carolina. Due to the distance from North Carolina, you’ll complete your assessment and recommended treatment in close proximity to where you live rather than in North Carolina.

What you need to do

Once your assessment and treatment are completed in your home state,Call (980) 269-8935 for Monroe office and (704)668-9017 for the Charlotte office to schedule an appointment with Smart Choice for an over-the-phone assessment.

What Smart Choice will do

After the phone assessment, if no additional treatment is needed, a Smart Choice expert will complete an E-508 which states you fulfilled your treatment requirements. Smart Choice will then submit the E-508 form to the North Carolina Department of Health and Human Services and the North Carolina DMV.

When the E-508 is approved, any block preventing you from gaining full driving privileges will be removed.
